All-on-4 achieves equivalent stability with four implants by tilting the two posterior implants at an angle (typically forty-five degrees) rather than placing them vertically.

First, it maximizes contact between the implant and available bone by using a longer implant trajectory through a denser region of the jaw. Second, it avoids the areas of the posterior jaw where bone is typically thinner and where sinus anatomy in the upper jaw limits implant depth. Bone grafting has historically been one of the major barriers between patients and dental implants. When bone loss has occurred after tooth extraction — as it invariably does over months and years — there may not be enough bone depth or density to place implants. Traditional implant protocols often required bone grafting and months of healing before implants could even be placed. All-on-4’s angulated design sidesteps this barrier for most patients, compressing what could have been an eighteen-month process into a single surgical day.

The procedure itself is performed under local anesthesia, with sedation options available to ensure comfort for patients who experience dental anxiety. Any remaining failing teeth are extracted, the implant sites are prepared, and all four implants are placed in a single surgical appointment. A temporary full-arch prosthesis — a complete set of teeth — is attached to the implants the same day, so patients leave the office with a functional smile.

Over the following three to six months, the implants undergo osseointegration — the process by which the titanium surface fuses permanently with the surrounding bone. During this period, patients wear the temporary prosthesis and attend follow-up appointments to monitor healing. Once osseointegration is confirmed, the final permanent prosthesis is fabricated and placed.

The final restoration is typically made from high-quality zirconia, chosen for its strength, stain resistance, and natural appearance. Zirconia withstands the forces of a full bite, doesn’t discolor over time, and creates a result that looks and functions remarkably like natural teeth.

How All-on-4 Compares to Conventional Dentures

The comparison is not a close one, and patients who have made the transition from dentures to All-on-4 describe the difference in terms that go well beyond functional improvement.

Conventional dentures rest on the gum tissue and are held in place by suction and, often, adhesive. They can slip during eating, create clicking sounds during conversation, and must be removed every night for cleaning. They also fail to address the progressive bone loss that continues after tooth extraction — because there are no implant roots stimulating the jawbone, the ridge gradually resorbs over years, changing the fit of the denture and altering the facial profile in ways that accelerate visible aging.

All-on-4 implants are permanently fixed — they cannot be removed by the patient and function as a structural part of the jaw. Bite force is restored to a level approaching that of natural teeth. Patients eat the foods they avoided with dentures.
